CLINICAL CASE #021: ENGINEERING RESTORATION

Contact Point Restoration (Class II) | Teeth 2.3, 2.4

1. CLINICAL SITUATION

  • Location: Upper left arch, Canine (2.3) and First Premolar (2.4).
  • Defect Type: Class II (Black) on proximal surfaces.
  • Task: Anatomical reconstruction of the contact point and marginal seal.
  • Registry Note: This case demonstrates the performance of the Quickmat™ system in the frontal-lateral transition zone (2.3–2.4), where contact geometry requires precise control of matrix curvature.

2. RSBC PROTOCOL (Step-by-Step)

Rubber dam insulation

Nictone™

Clamps for fixation

TOR VM™

Best matrix system

Polydentia™

  • R (Rubberdam): Isolation of the operative field using the NicTone™ system. Stability is ensured with Tor VM™ clamps. The use of a medium-grade sheet guarantees the absence of sulcular fluid and respiratory moisture in the bonding zone.
  • S (Sandblasting): Kinetic substrate preparation ($Al_2O_3$, 27 $\mu m$, 2.0 bar). Micro-abrasive treatment is aimed at removing the “smear layer,” desquamating defective tissues, and activating enamel prisms to enhance micromechanical retention.
  • B (Bonding): Adhesive stage using Kerr Optibond FL™:
    • Total-Etch: Etching with 37% Phosphoric Acid (enamel/dentin).
    • Infiltration: Active scrubbing of primer and filled adhesive (4th generation).
    • IDS (Immediate Dentin Sealing): Immediate sealing of the dentin creates a reliable biological barrier and increases bond strength.
  • C (Composite): Biomechanical reconstruction. Restoration of the proximal wall using the matrix system, followed by incremental layering of the occlusal morphology.

myWedge™ Interdental WedgesREF 7102Plastic adaptation and cervical margin sealing

4. ENGINEERING BUILD-UP

  • Mechanics: Utilization of the myRing Forte™ separation ring for controlled opening of the interproximal space.
  • Adaptation: Positioning of myWedge™ to prevent the formation of overhangs in the critical cervical zone.

5. TECHNICAL VERIFICATION

  • Video Protocol: #8KDCV – 8K Dental Clinical Video.
  • Format: 8K HDR.
  • Operator: Alexander Apokin
  • Location / Date: Moscow, 2023.
